When patients have extensive recurrent stricture and completely scarred distal urethra and desires a simple solution to relieve the obstruction, a perineal urethrostomy is an option.
Perineal urethostomy is an opening of proximal urethra under the scrotum. During urination, the urine exits from the urethrostomy. This procedure is really helpful in older patient with multiple medical problems who desire simple and definitive treatment.
It’s not a good option in young patients, as they do not want to sit to urinate or have semen coming out from under the scrotum.
If this procedure is not done properly it can lead to stenosis and complete blockage of proximal urethra.
